Yale New Haven Health Clinical Affiliates (YNHHCA) is a physician-led and physician-governed clinical network of independent physicians, physicians from Yale Medicine and Northeast Medical Group, community partners, and Yale New Haven Health System (YNHHS), collaborating in a shared vision for healthier patients, families and communities.
Member physicians and YNHHS seamlessly share information and resources to offer an integrated healthcare experience for our patients. Members can rely on our enterprise's clinical and administrative expertise and nationally recognized academic health system to help provide the highest quality care, enhance care management services and succeed in value-based arrangements with payors. Members will be well supported and will work most closely with Yale New Haven Health's Center for Population Health to facilitate the coordination.

Yale New Haven Health Clinical Affiliates (YNHHCA) is a physician-led network. The organization’s board of directors includes four Yale School of Medicine physicians, four Northeast Medical Group physicians, and three standing members representing Yale New Haven Health administration.
