Our clinically integrated network includes Yale Medicine, Northeast Medical Group, Yale New Haven Health Urgent Care (formerly PhysicianOne Urgent Care), and Health @ Home. It also includes Yale New Haven Health System hospitals, including Greenwich Hospital, Bridgeport Hospital, Yale New Haven Hospital, Lawrence + Memorial Hospital, and Westerly Hospital in Rhode Island and their associated ambulatory facilities.

Leveraging input from our academic and community clinicians, our Yale Care Signature pathways support evidence-based best practice dissemination.

Our demonstrated success in value-based care allows us to pass along shared savings with member physicians and support member physicians in Medicare's QPP, PCMH, and other quality program submissions.

Quality
As a clinically integrated network, YNHHS Clinical Affiliates, with the approval of its Board, agrees upon a standard set of measures for all participants to focus on improving and enhancing the health of the populations and communities we serve.
YNHHS Clinical Affiliates quality program will define, measure, track and report these metrics across participants, and our Center for Population Health supports our practices to improve performance, quality and patient health outcomes successfully.
Population Health
Yale New Haven Health Clinical Affiliates (YNHHCA) focuses on providing high-quality, cost-effective health care tailored to the needs of patients and families. The network is designed to engage patients in their care and ease the stress of navigating the healthcare system while achieving better health outcomes.
Our members have access to advanced care coordination programs, improved communication capabilities, and connections to community programs that can provide additional support to our patients. Programs are informed by our network's advanced data and analytics program, which provides deeper insight into the care of shared patients.
