CIN Board and Leadership

Yale New Haven Health Clinical Affiliates (YNHHCA) is a physician-led and governed network. The organization's Board of directors includes members from its current constituent entities: Yale School of Medicine, Northeast Medical Group, and Yale New Haven Health administration. The Center for Population Health leadership supports the operational work of the clinically integrated network.

Rajlakshmi Krishnamurthy

Rajlakshmi Krishnamurthy, MD

Associate Dean of Population Health, Chief Population Health Officer

Dr. Krishnamurthy is the Associate Dean of Population Health for YSM and SVP and Chief Population Health Officer for YNHHS. Dr. Krishnamurthy is responsible for improving value-based care and the sustainability of healthcare delivery through collaborative partnerships with physicians, system hospitals, payers, community-based organizations, and preferred partners. She drives the strategic development of population health efforts and programs, clinical integration in YNHH Clinical Affiliates, physician performance improvement, and ambulatory quality and value.

Polly VanderWoude

Polly VanderWoude, MHSA, FACHE

Executive Director, Population Health & Clinical Integration

Ms. VanderWoude develops and executes the system's strategy for succeeding in population health management and value-based care. She leads innovative program development to enhance health outcomes and evaluate and implement payment models that align incentives across providers, payers, and consumers.

Ohm Deshpande

Ohm M. Deshpande, MD, MHA

Associate Chief Population Health Officer & Vice President, Clinical Finance

Dr. Deshpande has a wide range of healthcare experience, focusing on population health, clinical operations, care management, quality improvement, healthcare finance and reimbursement, and managing relationships with payers and employers. Dr. Deshpande currently holds strategic leadership positions at Yale New Haven Health. He has led key initiatives to assemble components of our population health support systems as the health system has evolved into an integrated enterprise.

 Anita S Arora

Anita Arora MD, MBA, MHS

Associate Chief Population Health Officer and Associate Professor, Medicine

Dr. Arora has served in leadership roles within Yale Medicine population health and clinical operations, and she has experience implementing population health management strategies focused on improving access, quality, efficiency, and coordination of care. Dr. Arora clinically practices medicine and teaches residents and medical students at the New Haven Primary Care Consortium.  

Board Members

As a physician-led organization, Yale New Haven Health Clinical Affiliates includes on its 12-member Board of Managers Yale School of Medicine physicians, Northeast Medical Group physicians, and standing members representing Yale New Haven Health administration (the chief clinical officer, the chief physician executive, the chief financial officer, and the chief population health officer).

Sub-Committees of the Board include IT and Analytics, Quality and Performance, Network Management, Contracting and Finance, and Nominating.
