Our certified medical staff provides medical services at home following a hospital stay or for anyone with an acute condition or exacerbation of a chronic condition. Working with the patient’s doctor, we will help improve the quality of life by creating and carrying out a healthcare plan to meet the patient’s exact needs.
Our care team helps clients with activities of daily living (ADLs) such as dressing, bathing and transferring as well as meal prep, light homemaking and transportation to appointments. Non-medical care givers do not help with clinical needs but provide crucial services to help our clients live their best lives.
We are dedicated to providing choices, control and comfort to our patients, and committed to supporting caregivers. We offer personal and medical care enhanced by therapies to address all needs ranging from those of the body to those of the spirit and mind.
