Overview

The Heart and Vascular Center's Comprehensive Heart Failure program provides evaluation, management and treatment for patients with heart failure, a disease of the heart muscle which does not allow the heart to adequately support the rest of the body's functions. Our goal is to provide each patient with a tailored treatment plan addressing a patient's physical, mental and emotional needs with nurse-led clinics, telemanagement, nutrition counseling and social services.

Our multidisciplinary team is composed of specialists representing every aspect of cardiac care, including heart failure cardiologists and cardiac surgeons, as well as dedicated advanced practice providers, nurse coordinators, pharmacists, psychologists, transplant immunologists, dietitians, financial counselors, palliative care specialists and social workers. They are often among the first in the country to use new medicines, procedures and devices for every stage of heart failure.

Our goal is to provide each patient with a tailored treatment plan addressing a patient's physical, mental and emotional needs with nurse-led clinics, telemanagement, nutrition counseling and social services.

What Is Congestive Heart Failure

Congestive heart failure (CHF) is a type of heart failure and a chronic condition that can be caused by high blood pressure, hypertension, heart attacks, heart infections and heart valve problems. Common symptoms can include:

  • Fatigue
  • Inability to walk or exercise
  • Leg swelling
  • Shortness of breath

  • Acquired cardiomyopathies diseases (amyloidosis, sarcoidosis)
  • Acute decompensated heart failure and cardiogenic shock (amyloidosis, sarcoidosis)
  • Advanced or end-stage heart failure

  • Complex coronary artery disease and heart valve disease
  • Complex valve and coronary disease
  • Heart failure with preserved ejection fraction
  • Inherited cardiomyopathies
