Inpatient
Anita Jacobs, RN, North Pavilion Operating Room, was recognized for practicing a questioning attitude when she noticed an unattended shipping box and paperwork in the OR. Upon investigation, she discovered that a patient had made arrangements to preserve a specimen to send to an out-of-state lab for further testing. Jacobs questioned the process of sending a specimen outside the hospital and clarified the handoff process with the surgeon and Pathology to ensure the specimen was properly handled and sent.
Outpatient
Joyce Albert, senior clinical transplant coordinator, kidney/pancreas, was recognized for attention to detail when she tried to contact a living kidney donor. Using the phone number listed in Epic, she instead reached another patient. Albert stopped the line and escalated her concerns. An investigation revealed that the living donor and the patient Albert had contacted had the same name, middle initial and date of birth, but lived in different states. The transplant coordinator team worked with the Patient ID team and Health Information Management to rectify the situation and flag both medical records to avoid future confusion.
