A multidisciplinary team from the Department of Emergency Medicine was honored for executing an innovative care model for a patient transitioning to hospice care at home. The patient needed to be intubated for transport, then extubated at home. Over a weekend, emergency physicians coordinated with the inpatient team and ambulance crews to ensure a safe plan for discharge and provide dignified and compassionate care. Amelia Breyre, MD, accompanied the EMS crew in the ambulance that transported the patient home, then performed a palliative extubation in the patient’s home before transitioning her to hospice care. Dr. Breyre led the multidisciplinary team, which also included Medical ICU staff, residents, physicians and staff from YNHH’s Center for EMS and YNHH management.
