Yale New Haven Health is committed to improving the health of the communities we serve. The YNHHS Office of Health Equity and Community Impact (OHECI) routinely collects information about the main health concerns of residents through a triennial process called the Community Health Needs Assessment (CHNA).
Informed by residents and community leaders, YNHHS collaborates with partner organizations to create strategic initiatives and enhance services that build healthier communities.
Data collected for the CHNA come from multiple sources, including residents, local experts, federal/state information and community organizations. Each YNHHS delivery network conducts its own CHNA to ensure programming addresses the distinct needs of each community.
“Health is more than health care. A person’s ability to lead a healthy, fulfilling life is impacted by where they live and social drivers of health such as economic stability, education, safe housing, healthcare access and food security,” said Nancy Hamson, director, Community Health, OHECI.
CHNA results are used to identify our communities’ top needs, then evaluated to create a three-year plan called a Community Health Improvement Plan (CHIP).
