David Vinas, director of Operations, Smilow Cancer Hospital, demonstrated attention to detail when he observed that a piece of clinical equipment had not been readied for use using proper practice guidelines. He stopped the line and removed the equipment. His actions led to a policy revision on equipment maintenance and the education and supervision of new residents.

Before administering an antibiotic to a patient, Antonio Onofrio, RN, Emergency Department, practiced a questioning attitude. Although the patient had denied having medication allergies, Onofrio checked an old electronic medical record system and found the patient had an anaphylactic allergy to another drug in the same family. He updated the Epic record to reflect this information and immediately informed the physician, who changed the order.
