Elise McCormick, clinical pharmacist, York Street Campus Central Pharmacy, was recognized for practicing a questioning attitude, 200% accountability and attention to detail. McCormick was assisting with a medication reconciliation for a patient who was set to resume their home medications. She noted several discrepancies between the medications listed on the handoff and the patient’s filled prescriptions. She contacted the patient’s daughter and learned that the pharmacy associated with these medications was in Florida. Further investigation showed the patient’s birth year was entered incorrectly. McCormick ensured that the patient’s information was accurate and the entire medication list was correct.
Elisa Gagliardi, RN, Saint Raphael Campus MICU, was recognized for practicing a questioning attitude, attention to detail and 200% accountability that led to a systemwide Epic update. A fellow nurse asked Gagliardi to help manage a patient who had become hypoglycemic while on a regular insulin infusion. Gagliardi noted that the incorrect insulin infusion order had been selected due to an inappropriate “reason for use,” which produced an inaccurate rate. She escalated her concerns to Pharmacy Services, which determined the need for an Epic medication order set optimization. The optimization is now live and will prevent future events.
