Samya Skourides, surgical technician, Saint Raphael Campus, was recognized for practicing a questioning attitude, 200% accountability and attention to detail. When a drill was handed to her during a procedure, Skourides noted that the blade was broken. She provided a crosscheck to the team, and an X-ray showed the remainder of the broken blade. The item was successfully removed with no patient harm.
Mariangel Rodriguez, laboratory processing coordinator, Saint Raphael Campus, was recognized for attention to detail, practicing a questioning attitude and 200% accountability. Rodriguez used STAR (Stop, Think, Act, Review) and detected specimen-labeling errors on specimen containers for three different patients. She stopped the line and escalated her concerns, prompting additional steps to correct all specimen IDs and provide accurate pathology reports to the three patients.
