Great Catch winners help YNHH become a high reliability organization

Rachel Bates, RN, Medicine (EP 10-7), was recognized for attention to detail, practicing a questioning attitude and escalating her concerns while caring for a patient with respiratory failure. Although the patient appeared to be comfortable, Bates noticed a slight change in vital signs. She also noted that some blood test results were elevated from previous results. Suspecting a blood clot, she escalated her concerns to the attending physician. When a CT scan revealed multiple clots in the lungs, the patient received timely treatment to prevent a potentially life-threatening condition. 

Swani Schoberth-Wyckoff, RN, Pediatric PACU, was recognized for attention to detail and practicing a questioning attitude with a patient who arrived in the OR to have a baclofen pump placed. Noting that the pump would be placed right next the patient’s pacemaker, Schoberth-Wyckoff provided a cross-check and asked if the baclofen pump would interfere with the pacemaker. After validating and verifying, the surgical team determined that the two pumps would have nullified each other if they were too close together. The placement plan was adjusted, and the patient’s procedure was completed safely.
