YNHHS, VNA Community Healthcare & Hospice announce integration
Visiting Nurse Association Community Healthcare & Hospice (VNACH) recently integrated with Yale New Haven Health System, expanding patients’ home health care options.
VNACH is now part of Home Care Plus Inc., YNHHS’ community-based, home-health agency.
“Our health system’s hospitals and medical offices rely on expert home health to provide patients with the extended nursing care and therapies they need for recovery after illness or injury, or to manage patients before hospitalization in the comfort of their homes,” said Kathleen O’Leary, RN, vice president, YNHHS Post-Acute Care Services. “VNA Community Healthcare & Hospice has long been known for its excellent patient and whole-family care, which aligns with our mission and vision.”
The integration of VNA Community Healthcare & Hospice with Yale New Haven Health will expand high-quality home health options for people throughout the Shoreline region and help the health system better manage patients who can be safely and appropriately cared for at home.
Healthcare delivery has never evolved more rapidly than during the past two years. To expand services and provide the full continuum of care for patients, YNHHS has looked to partner with strong organizations that provide outstanding care in this evolving landscape. As YNHHS’ patient population is more inclined to age in place, this integration enhances the health system’s ability to provide coordinated, high-quality home health services.
VNA Community Healthcare & Hospice has been in operation since 1910; it currently serves 35 towns throughout the Shoreline region.
