Do you Know Your Numbers?
Yale New Haven Health is offering all employees free, onsite biometric health screenings at all delivery networks through Know Your Numbers (KYN) Plus. A biometric screening provides a snapshot of your physical health so you can identify and address risk factors such as high cholesterol, high blood pressure or elevated blood glucose levels.
For benefits-eligible staff who intend to enroll in the YNHHS medical plan for 2022, participation in a biometric screening earns $200 credit toward your 2022 premium and is required to earn up to an additional $300 in Thrive personal choice activities credit.
Onsite biometric screenings are conducted by YNHHS’ trusted vendor, Impact Health. Appointments are required. To schedule, click the link in the KYN Plus article on the intranet homepage, then, select “ImpactHealth-Prod” from the dropdown menu, “Sign In,” then enter your network credentials to access the scheduler.
Benefits-eligible employees may also receive a biometric screening from their healthcare provider or at a CVS Caremark location. To access the required form and instructions for uploading documentation, log in to the Virgin Pulse Employee Well-being Portal, located on the intranet under “Resources.” If you haven’t yet enrolled in Virgin Pulse, visit join.virginpulse.com/ynhhs.
All employees are encouraged to participate in YNHHS’ vast array of activities to support their well-being. As part of Thrive, the new hub for all YNHHS employee well-being resources, KYN Plus now includes an expanded list of personal choice activities, extending into areas such as emotional, social and financial wellness, to help bring your health and well-being full circle. While employees can use the many Thrive resources year-round to enhance their overall well-being, the KYN Plus component of Thrive runs through Sept. 15. LWC is a free and confidential care management program for Yale New Haven Health employees and their families. LWC helps participants make lasting lifestyle changes to improve health and well-being. Through care coordination, health and nutrition coaching, stress management, pharmacy support and more, LWC can help you manage potential health risks your biometric screening may have uncovered.
In addition, three visits with LWC can earn YNHHS benefits-eligible employees $300 in Know Your Numbers Plus 2022 medical premium credit if completed by Sept. 15, 2021. A biometric screening combined with LWC participation equals the maximum $500 KYN credit. To complete three LWC visits before Sept. 15, coaching must begin no later than Friday, July 30.
