Great Catch winners help YNHH become a high reliability organization

Congratulations to these Great Catch winners for modeling HRO safety behaviors and taking action to improve patient safety.
Outpatient
Linda Mesham and Nadina Wolf, radiology technologists, Diagnostic Imaging, were recognized for 200% accountability, attention to detail, speaking up for safety and practicing a questioning attitude while caring for a patient with a traumatic injury at North Haven Medical Center. Wolf and Mesham suspected the X-rays indicated a spine issue, so they escalated their concerns up several levels. The ordering provider requested a CT scan, which showed the patient had a spinal fracture. Because of Mesham’s and Wolf’s persistence and use of numerous CHAMP behaviors, the patient was correctly diagnosed and treated.
Inpatient
Katie Howe, RN, nurse coordinator, Pediatric ICU, was recognized for practicing a questioning attitude and attention to detail using STAR (Stop, Think, Act, Review) while caring for a critically ill patient. Howe discovered an issue that made her question the sepsis diagnosis. She escalated her concern and brought the team together to huddle. The patient was subsequently diagnosed with toxic shock syndrome and immediately treated.
