Great Catch winners help YNHH become a high reliability organization

Congratulations to these Great Catch winners for modeling HRO safety behaviors and taking action to improve patient safety.

 

Inpatient

Jeff Peterson, RN, Neuroscience ICU, was recognized for attention to detail while reviewing a patient’s MRI safety sheet. Peterson stopped the line when he noticed an access device still in place in a bone. This device contains metal, which is incompatible with the MRI procedure. Peterson escalated his concern to the provider, who removed the device and replaced it with a central line. Thanks to Peterson’s use of CHAMP behaviors, the patient was safely scanned.

Outpatient

Robert Psoter, CT technologist, Diagnostic Imaging, was recognized for attention to detail, practicing a questioning attitude and speaking up for safety after noticing that the radiation dose on certain imaging exams had suddenly increased. Psoter immediately escalated his concerns to the chief technologist and medical physicist teams, who contacted the manufacturer. An investigation showed that a software upgrade had unexpectedly increased patient doses. Psoter’s use of the CHAMP behaviors ensured that patients were not exposed to unnecessary levels of radiation.
