Congratulations to these Great Catch winners for modeling HRO safety behaviors and taking action to improve patient safety.

Outpatient
Monica Dwake, RN, Pediatric Primary Care Center, practiced a questioning attitude and escalated her concerns about an infant who missed an appointment. Dwake contacted the Visiting Nurse Association for follow-up, then escalated her concerns to the state after a second missed appointment. The infant came to the ED with life-threatening injuries and was admitted. Dwake’s questioning attitude prevented additional harm to this young patient.

Inpatient
Gail Macholl, patient care associate, Adult PACU, showed attention to detail while taking vital signs on a pre-op endoscopy patient. While she was reviewing the patient’s name and procedure, a provider entered to mark the patient’s extremity for surgery. Macholl stopped the line and practiced 200% accountability when she reviewed the procedure for ths patient, who did not require extremity marking.
