Morning Safety Reports are now getting a RISE out of staff

In early September, Yale New Haven Health introduced a new report in the morning safety huddles that occur throughout the health system. Called RISE (Resilience In Safety Events), the report is provided each Thursday by YNHHS safety specialists.

During the daily morning safety huddles, participants discuss safety events and near misses that occurred in the past 24 hours; anticipate safety or quality issues for the next 24 hours; and update status reports on previously identified issues. The goal of the RISE report is to provide more timely and actionable safety information at the huddles, said Stephen G. Jones, MD, YNHHS medical director of Safety.

Each RISE report focuses on:

  • Providing real-time updates on the system’s safety performance improvement initiatives (such as falls prevention, oxygen safety and others) and promoting and reinforcing key drivers of those initiatives
  • Ensuring validation and verification of level 2 safety alert communication and closure
  • Communicating pertinent external safety information (such as Joint Commission sentinel event alerts, Connecticut Hospital Association programs, etc.)
  • Providing feedback on completed significant safety events to identify lessons learned and corrective action plans

“Resilience is a critical component of any high reliability organization and includes the ability to adapt to a stressor, recover and become stronger through lessons learned,” Dr. Jones said. “As a measure of the power of organizational (and individual) resilience, we need only look at our health system’s remarkable and successful response to the COVID-19 crisis. The RISE reports enable us to begin looking at our safety events, and the learning opportunities they afford, with a greater focus on such resilience.”
