Leaders from Yale New Haven Hospital, Yale School of Medicine, Cornell Scott-Hill Health Center and Fair Haven Community Health Care participated in a ribbon cutting ceremony to commemorate the new Primary Care Medical Center at 150 Sargent Drive.
New partnership enhances primary care
Yale New Haven Hospital, Cornell Scott-Hill Health Center and Fair Haven Community Health Care celebrated the opening of a newly constructed Primary Care Medical Center at 150 Sargent Drive, in New Haven’s Long Wharf area, with an Oct. 20 ribbon cutting.
In November 2017, YNHH, Cornell Scott-Hill Health Center and Fair Haven Community Health Care announced a New Haven Primary Care Consortium designed to broaden access to primary care services by combining the three organizations’ resources and expertise.
“Together, with our partners at Cornell Scott-Hill Health Center and Fair Haven Community Health Care, we have joined efforts to improve the health status of the underserved population of New Haven and the surrounding communities. This new, extraordinary facility will support our efforts and our mission,” said Cynthia Sparer, senior vice president, Operations, YNHH, and executive director, Yale New Haven Children’s Hospital and Women’s Services.
“We’ve come to this milestone after literally thousands of hours of work, spanning more than five years,” said Suzanne P. Lagarde, MD, chief executive officer, Fair Haven Community Health Care. “The end result is an unprecedented opportunity to provide high-quality care for the majority of New Haven’s low-income and minority residents.”
Healthcare providers and employees from YNHH’s Adult, Women’s and Pediatric Primary Care Center locations at the York Street and Saint Raphael campuses, and Chapel Pediatrics in Hamden moved to 150 Sargent Drive, where they continue to provide patient care. Cornell Scott-Hill Health manages operations for adult and women’s health care; and Fair Haven Community Health manages operations for pediatric and adolescent health care.
Among the consortium’s many benefits are an expanded care model for patients, which includes integrated behavioral health services; on-site imaging and blood draw provided by YNHH; improved care coordination, with all three organizations using Epic for patient medical records; and an enhanced learning experience in community health care for YNHH residents and interns.
