Primary care practices earn Patient-Centered Medical Home recertification

The National Committee for Quality Assurance (NCQA) has awarded Patient-Centered Medical Home (PCMH) recertification to 44 Northeast Medical Group primary care sites. The nationally recognized NCQA is the most widely used vehicle to transform primary care practices into medical homes.

A PCMH uses a team-based approach to meeting patients’ healthcare needs. Led by a primary care physician, the team may include physician assistants, advanced practice registered nurses, medical assistants, nurses and care coordinators. The medical home model delivers comprehensive, personalized care in a way that is accessible to the patient.

To obtain recertification, each practice must maintain standards set forth by the NCQA and promote a positive impact on patient care, said Twila Balint, RN, NCQA PCMH coordinator for Northeast Medical Group.

“PCMH standards focus on the vital aspects of care, and they provide positive direction for achievable goals and ultimate health success,” she said. “PCMH recertification is not only a certificate to hang on the wall, but a symbol of the commitment of our staff and Yale New Haven Health to quality care for our patients.”
