Yale New Haven Hospital

Carla Hannon, RN 

Carla Hannon, RN, clinical nurse specialist, Saint Raphael Campus MICU, was recognized for attention to detail and 200% accountability. She noted that the stocked syringes for the bedside dialysis machine in her unit were not compatible. Recognizing that medications may not infuse at the correct rate with an incompatible syringe, Hannon alerted Supply Chain to ensure that machine-compatible syringes are available at all locations using this type of equipment throughout the health system.

Cheryl Handy and Ivan Foskey 

Cheryl Handy and Ivan Foskey, patient relations specialists, were recognized for their questioning attitudes while rounding in the York Street Campus Emergency Department. They saw a person heading into the ED Annex in apparent distress but he had stopped moving. They approached him and asked if he was okay. When the man began to lose consciousness, Foskey caught him and helped him safely to the floor while Handy flagged down a nurse manager to assist. Thanks to their use of CHAMP behaviors, Handy and Foskey prevented a potential fall with injury.

Greenwich Hospital

Carlos Charles, RN 

Carlos Charles, RN, Emergency Department, had a patient who had nausea, vomiting and diarrhea for 24 hours. The patient was alert and oriented when first assessed, but Charles noted slurred speech and facial droop during a reassessment an hour later. He immediately activated a stroke code and escalated his findings to the provider. The patient was diagnosed with acute onset stroke, given medication and transferred to another hospital for thrombectomy. Charles’ excellent assessment, communication and hand-off skills saved his patient from a serious outcome.
